Dementia Care Intake Process
When you are ready, we will plan a meeting with you and our Community Manager to create a care plan. Prior to this meeting, we will send you an electronic intake form to gain a better understanding of your needs. This form allows us to focus on creating a plan that works best for you and your situation so we can discuss your next steps.
If you, your ageing parent, or your loved one needs dementia care, we will make it our priority to find an experienced caregiver that you can trust.
