Our Senior Home Care Intake Process
When possible, we will send you the form electronically before you speak with a care provider so you can start filling out the information and we can focus on what matters to you during our meeting. The intake form is quite comprehensive, but you only need to share what you are comfortable sharing with us.
The more information we have, the better we can build your custom care plan and pair you with a health care aide or licensed practical nurse that will suit you and your family’s needs and preferences!

When returning from the Hospital
Going from round-the-clock assistance to being alone can be scary and unsafe. HCAs and LPNs can help you cope with your return and set you up for a speedy recovery.
When you or your loved one has an unexpected fall
Daily check-ins, help around the house, company, driver to go to social activities, errands, regular home risk consultations, presence when contractors come in (electricians, plumbers, landscape, etc.), help with technology, assistance with activities of daily living, accompaniment to medical appointments.
When independence is at stake
Respite for the primary caregiver, intellectual stimulation, companionship during social activities, help around the house, guidance for the senior living with dementia to assist with chores, transportation to and from a day program.
When care is becoming too much for the primary caregiver
If family caregivers hurt themselves or are becoming increasingly tired and frustrated, there are now two clients. Ohana can care for the household and assist with chores to alleviate stress and give everyone time to enjoy each other’s company!
When the primary caregiver is going away
Ohana can send caregivers for short-term contracts as well.
When learning of a new diagnostic or going through new treatments
New medication may have undesirable side effects, and it’s not always possible to do wellness checks daily. Our HCAs and LPNs can help by monitoring moods, skin, vitals, balance, and more. They can also help care for you if you get very weak, have severe nausea, are at risk of fall, or if your judgment is impeded.

Typically, a Health Care Aide will spend more time with a client, getting them ready for the day, doing all morning care, assisting with breakfast and providing medication reminders. Our Health Care Aides also support the lifestyle of our clients by doing laundry, changing the beddings, respecting their regular routine, and being a companion using the Best Friends Approach™.
Health Care Aides are also used as companions when clients have “Unscheduled Care Needs”, meaning they will need support throughout the day that cannot be scheduled. These include incontinence, PRN medication (headaches, aches, constipation, diarrhea), transfers, asthma pumps, and more.

Licensed Practical Nurses are more medically focused in their work with the clients. They may only spend one hour per visit simply to make sure there aren’t any drastic changes in the health of the client, ensure the HCAs are following the care plan, assist with more complex care needs, and assess the overall wellbeing of the client. LPNs may only visit a client right after they are back from the hospital, or when a new care plan is to be put in place.
During our first meeting, we will determine together what is best for you and your family. We will take into consideration your gender, medical conditions, and surrounding needs. Please let us know right away if you are not happy with the caregiver coming into your home. You should also feel safe, respected, and know you are in control.
Absolutely! We will also make sure to invoice you in the way that makes most sense to you to alleviate the stress of keeping track of all the invoices. We can print them, mail them, email them, and retroactively issue new ones.
We always charge for at least 3-hours, but we encourage you to use that time to its fullest potential. We can do a home safety check, clean the dishes, fold clothes, go over items in the fridge to ensure nothing is passed its shelf life, help the client make phone calls, assist with typing emails, and other basic care needs. However, some exceptions may apply to the minimum hours. Book your free assessment to find out more.
- A binder can be left at the home of the client and state what care was provided and if there are any behavioral changes. This is the best option if you have multiple caregivers and family members.
- Our care givers can call you from the phone at your loved one’s home and give you a run down before they leave (If you have Power of Attorney, or if a release of information form has been signed).
